Eligibility Criteria

Housing Stabilization Services are available to Minnesota residents who meet these criteria:

Age Requirements Adults 18+ with disabilities or seniors 65+ are eligible for our services.
Medical Assistance Must be enrolled in Minnesota Medical Assistance (Medicaid) program.
Housing Status Currently homeless, at risk of homelessness, or living in institutional settings.
Assessment Complete a person-centered assessment to determine your specific service needs.
Provider Network Services must be provided through an enrolled HSS provider in Minnesota.
